Abstract: Congenital heart diseases represent a significant health concern in the neonatal period, being associated with high morbidity and mortality and often requiring early surgical intervention. In this context, the perioperative period poses important challenges, particularly regarding cardiorespiratory complications. Physical therapy plays a key role in this scenario, contributing to preoperative preparation, ventilatory management, and prevention of postoperative complications. This study aimed to analyze the role of physical therapy in the preoperative and postoperative care of neonates undergoing cardiac surgery due to congenital heart disease. This is an integrative literature review with a qualitative approach, including studies published between 2016 and 2026 that were available in full text and related to the topic. The findings indicate that physical therapy is associated with improved respiratory function, reduced duration of mechanical ventilation, and lower incidence of pulmonary complications. However, variability in approaches and lack of standardized protocols were observed. It is concluded that physical therapy plays a relevant role in neonatal perioperative care, although further studies with greater methodological rigor are needed to strengthen evidence-based practice.
